We respect your family’s choice about being featured in Heart Beats photos, recordings, and school materials.

Complete this form to let us know that you do not want us to share images, recordings, names, or comments that identify you or your child. Your choice will not affect participation in our programs, and it will stay on file unless you change it in writing. Photos or recordings may still be taken during school activities.

Please submit one form per person. For a child under 18, a parent or legal guardian must submit the request.

If something already shared concerns you, please include a link or description below. Allow up to 30 days for review and removal or replacement. We prioritize urgent safety concerns; you are also welcome to contact the office directly. We cannot retrieve printed materials already distributed or guarantee removal of copies others have saved or shared.
