Use this form to request planned time off when you will be unavailable for all or part of your normal teaching schedule.

Please submit your request before changing or canceling affected students’ lessons, and wait for approval before making any changes to your schedule. Once we’ve reviewed your request, we’ll let you know how any affected lessons will be handled.
